2. Tax Info (W-9)
Required upfront since this is a formal 1099 contractor role. Your info is kept private and only used for tax reporting if you cross $600 in earnings this year.
3. Independent Contractor Agreement
By signing below, you acknowledge and agree that: (1) You are an independent contractor, not an employee of Site Doc LLC — you control how, when, and where you perform sales activities. (2) You are responsible for your own tools, expenses, and self-employment taxes. (3) This relationship is non-exclusive — you are free to sell for other companies. (4) You will be compensated solely on a commission basis (50% of build fees you close), with no salary, benefits, health insurance, or paid time off. (5) Either party may end this relationship at any time, for any reason. (6) You will receive a Form 1099-NEC for any calendar year in which your earnings meet the IRS reporting threshold.
